Geriatric Assessment

During the initial visit, our Geriatric Social Worker will evaluate existing abilities and needs through an interview, often with family present. The assessment will include an evaluation of health status, functional status, cognitive functioning, emotional status, environment, legal and financial situation, and family support system.

RECOMMENDATIONS AND SERVICE PLANNING

After completing the assessment the Geriatric Social Worker will set up a second visit to go over the assessment and make recommendations. The Geriatric Social Worker will then work with the older adult and or family to create a long term care plan for addressing their needs.

COORDINATION AND IMPLEMENTATION OF SERVICES

Once the plan is in place, the Social Worker can make specific arrangements to get services started. The Geriatric Social Worker will then follow up to make sure services are in place.

ONGOING MONITORING AND COORDINATION

Our Social Workers will work with you on an ongoing basis through follow-up visits and phone contacts. The Geriatric Social Worker can be available to monitor care and services, provide emotional support, stay informed, and respond as needed including be available if a crisis should occur.
